HACC-PYP

Is your independent living at risk?
HACC-PYP services are targeted to all people under the age of 65 years, who are living with moderate, severe or profound disabilities, and their unpaid carers.
What is HACC-PYP?
HACC-PYP services are targeted to all people under the age of 65 years old who are living with mild, moderate or severe physical or mental health limitations, and their unpaid carers.

Access and Support

Assistance with navigating through the health care system, and aids in wrapping services around clients that help maintain their independence.

Friendly Visiting Program

Social support individual engagement by matching volunteers with clients who, due to physical and/or mental limitations, illness, isolation or age, are unable to socialise and interact with the community. MCWA volunteers can accompany clients on shopping trips or doctor’s appointments, or simply chat over a cup of coffee.

Planned Activity Groups

Social support engagement in a group setting that can help clients meet new people, explore new locations, and try new things in a safe way.
Our groups help expand the social life of our HACC-PYP clients.

Are you eligible?

Are you under the age of 65 years old (and under 50 for Aboriginal or Torres Strait Islander people)?
Are you living with mild, moderate or severe physical or mental health limitations and not receiving NDIS funding?
Are you an unpaid carer of someone who is living with mild, moderate or severe physical or mental health limitations?

HOME CARE
PACKAGES

Servicing all of Australia, Home Care Package is a Government-funded program that gives the ageing and elderly the opportunity to access funding to support their decision to remain in their own home for longer.

IN HOME
SERVICES

through CHSP in the Northern and Western region of Melbourne, MCWA can assist with a range of domestic assistance, respite services, personal care, food services (delivered meals and centre-based meals) property maintenance in and around your home and more.

ALLIED HEALTH PROFESSIONALS

Through CHSP in the Southern region of Melbourne, MCWA connects clients and carers with Allied Health Services such as; Physiotherapy, Podiatry and Occupational Therapy.

SOCIAL SUPPORT GROUPS

Through CHSP in the Northern and Western region of Melbourne Conducted in a planned and supervised environment, SSGs are socialising groups hosted by MCWA and designed to give the ageing and elderly the facility to socialise.
